TERMS AND CONDITIONS

LIMITED LIABILITY WAIVER AND PHOTOGRAPHY CONSENT FOR ELEVEN ONE EVENTS

1. ASSUMPTION OF RISK I, the undersigned, acknowledge that participation in events organized by Eleven One involves inherent risks, including but not limited to physical injury, emotional distress, property damage, or other losses. I voluntarily assume all risks associated with my participation in any Eleven One event.

2. RELEASE OF LIABILITY In consideration of being permitted to participate in Eleven One events, I hereby release, waive, discharge, and covenant not to sue Eleven One, its officers, employees, volunteers, or agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any property belonging to me, whether caused by negligence or otherwise, while participating in such events.

3. INDEMNIFICATION I agree to indemnify and hold harmless Eleven One, its officers, employees, volunteers, and agents from any loss, liability, damage, or cost they may incur due to my participation in Eleven One events, whether caused by negligence or otherwise.

4. PHOTOGRAPHY AND MEDIA CONSENT I grant Eleven One, its representatives, and affiliates the irrevocable and unrestricted right to use, reproduce, publish, and distribute photographs, videos, or other media taken of me during Eleven One events for promotional, advertising, educational, or other purposes, in any medium, without compensation or further notice to me. I waive any right to inspect or approve the finished product.

5. EMERGENCY MEDICAL TREATMENT In the event of a medical emergency, I authorize Eleven One to secure medical treatment on my behalf, and I agree to assume all costs associated with such treatment.

6. GOVERNING LAW This agreement shall be governed by and construed in accordance with the laws of Newberry, SC. Any disputes arising under this agreement shall be resolved in the courts of Newberry County.

I certify that I am of legal age (or, if a minor, my parent/legal guardian has signed below) and have read and understood the contents of this waiver and consent form. I sign it freely and voluntarily, and it applies to all Eleven One events I participate in unless otherwise specified.
